The practice

Bentley Village Surgery is purpose built for a small number medical practitioners to provide general medical services to their patients Monday-Friday 8.00am - 6.30pm under contract to the North Hampshire Primary Care Trust. Out of hours cover is provided by Thamesdoc from 6.30pm - 8.00am every weekday night, and every weekend from 6.30pm Friday to 8.00am Monday.

These notes are designed to help you gain the most from these services.

The practice provides good access for disabled patients. If you are disabled or have any special requirements, please do not hesitate to indicate this to us. If you would like to discuss your disability with any of the medical staff, please make an appointment.

Information held about you by the practice will be kept in the strictest confidence and only shared on a need-to-know basis with other healthcare professionals; you may register an objection to this if you so wish. External staff who come into the practice will only be able to access named patient data with the explicit consent of the patient. Staff employed by the surgery adhere to the guidelines as stated in the Bentley Surgery Security Policy.

The Bentley Surgery has produced a Publication Scheme under the Freedom of Information Act 2000 - this can be viewed on this website or on the noticeboard at Reception.

Should the need arise, it is the practice policy to attempt to resuscitate our patients. If you have strong feelings regarding this please discuss the matter with the doctor. Any decisions relating to cardiopulmonary resuscitation will be recorded and relayed to all members of staff.
